Usually because it was submitted faster than the job expects, or the proof text matched something you sent on another task. Flagged does not mean rejected - it means the buyer is asked to look closely. Genuine work passes.

Report it from the task page. An administrator can see your proof, the reason the buyer gave and your time on the task, and can credit you if the rejection was unfair.

No, and you do not need to do anything. You may see a notice saying several accounts share your connection - that is us being open about what we record, not an accusation. Message support and it is noted on your account.

They are checked and paid by a person, so not instantly. The amount leaves your balance right away so it cannot be requested twice.
